Wayne Memorial Community Health Centers (WMCHC) has made connecting with free and reduced-cost community programs for essentials such as food, housing, childcare and transportation easier. Through its collaboration with the Keystone Accountable Care Organization (ACO), WMCHC added Neighborly, an online tool linked to an expansive data base of community agencies, accessible on its website www.wmchc.net. “Having an up-to-date list of contact information for vital services on one platform will prove beneficial for both our patients and staff,” stated Norma Nocilla, BSHA, LPN, CPHQ, clinical operations and quality director, WMCHC. Neighborly is easy to navigate and allows users to pinpoint agencies and services within their own community by simply entering a zip code. The search will generate a curated list of agencies along with next steps for the best way to connect with them ranging from phone numbers, direct website links to online forms. A Google map to the facility’s location is also part of the search. Neighborly is supported by Aunt Bertha, a public benefit corporation that provides the platform, identifies local, state and national resources.
