Community HealthChoices Events

Community HealthChoices (CHC) is the mandatory managed care program for Pennsylvanians who are dually eligible for both Medicare and Medicaid (Medical Assistance), enrolled in Medicaid waivers for physical disabilities, and Medicaid-eligible consumers who reside in a nursing facility.   This program was implemented in the southwest and southeast part of Pennsylvania over the past two years. As a managed care program, CHC gives individuals the choice of health plans to best match all their healthcare and long-term care needs. Community information meetings will be held in the morning and the evening across the Lehigh/Capital, Northeast and Northwest Zones for attendees to learn more about the CHC Program. PACE + Medicare Part D = Affordable Meds for Seniors 

Pharmaceutical Assistance Contract for the Elderly (PACE) is a comprehensive drug benefit that offers low-cost prescription medication to Pennsylvania residents who are age 65 and older. Benefits of being enrolled in both the PACE Program and Medicare Part D:

  • Coverage during the Part D Deductible Period & Donut Hole
  • Lower copays with some plans (generic specific)
  • Lower monthly premium vs. having to pay the monthly benchmark premium payment (specific to PACENET)
  • 90-day supply (if offered by the Part D plan)
  • Offset of program costs

Applications can be requested over the phone but are printable from the website and can be submitted online. Consumers can call 1-800-225-7223 or click here for a paper application or online application.
