CDO Application Refresh New Opportunity

Don't worry - If you did not complete your Certified Application Counselor Designated Organization (CDO) application for existing organizations through the Centers for Medicare & Medicaid Services (CMS), there is another opportunity to reapply. Even if you were not a CDO in the past, you have an opportunity to apply. The open season for applications is July 2, 2018 through Sept. 30, 2018. We strongly encourage all health centers to become a CAC/CDO using this application link. CDOs are a vital component of the assister community and allow your staff to train and aid your patients on the Health Insurance Marketplace, Medicaid, CHIP and other types of health coverage. Linking Community Health Workers to the Community

As health centers are becoming increasingly accountable for outcomes and health measures, community health workers (CHWs) play an important role in connecting those in the community to good, comprehensive care provided by health centers statewide. Not only do CHWs provide a needed personal touch to educational programs but they also become part of the local and regional safety net. CHWs meet people where they live, work, play and pray to address nutrition, health, transportation, housing and many other issues. FamiliesUSA has a Community Health Worker Sustainability Collaborative program to promote different strategies and better integrate CHWs into the healthcare system.
