PACHC will once again be hosting NCQA Patient-Centered Medical Home (PCMH) training for those health centers that are looking for their first recognition, those that are up for renewal, and those that would like a “refresher” or are new to PCMH and want to learn what it’s all about.

Health Center Medicare Savings Program/Low Income Subsidy Enrollment Program Training

Health centers are increasingly adding more needed services and becoming not only one-stop integrated healthcare homes, but also trusted resources for insurance and enrollment assistance. PACHC has entered into a partnership with the PA Department of Aging (PDA) to train enrollment assisters at health centers to provide enrollment assistance and enroll Medicare Beneficiaries eligible for the Low-Income Subsidy program (LIS) or the Medicare Savings Program (MSP) through intensified outreach and enrollment assistance. For each completed and processed enrollment application, the health center will receive $25.  This partnership with PDA will increase the ability to assist more patients to gain the security of insurance coverage in line with the Community Health Center mission and will also support health center financial strength by increasing the number of insured served.
