Pennsylvania Association of Community Health Centers serves as the collective voice for Pennsylvania's Federally Qualified Health Centers (FHQCs), FQHC Look-Alikes (FQHCLAs), and non-profit Rural Health Clinics (RHCs).  Our members are unique nonprofit primary care facilities that provide a wide array of services to Pennsylvanians in underserved areas of the state. There are nearly 300 sites in 52 counties serving over 800,000 Pennsylvanians.  At these primary care facilities, patients will receive high quality care at a price they can afford with sliding fees based on income and family size.

NNOHA/PACHC Integrating Oral Health into Primary Care Practice Collaborative

PACHC in collaboration with The National Network for Oral Health Access (NNOHA) is seeking interested Pennsylvania health centers to participate in a learning collaborative, Integrating Oral Health into Primary Care Practice (IOHPCP), funded through NNOHA's cooperative agreement with the Health Resources and Services Administration (HRSA). The goal of the IOHPCP Learning Collaborative is to improve health center patients' overall health by sharing experiences and promising practices in adopting and implementing oral health core clinical competencies (risk assessment, oral health evaluation, preventive interventions, communication and education, inter-professional collaborative practice) resulting in integration of oral health and primary care through inter-professional collaborative practice. The learning collaborative will take place between January 2018 and June 2018, with the first webinar being Jan. 23, 2018 at 3:00 pm; FREE CE will be offered. To be eligible, health centers need to provide both medical and dental services onsite and not through contracted services, and utilize and produce health outcome measurement reports from a fully implemented electronic medical record (EMR) and an electronic dental record (EDR). Minimal reporting is required. Please submit the questionnaire ASAP, as it needs to be received by PACHC for health center participation in any of the webinars.  For questions, please email Cheryl at PACHC.
