Pennsylvania Association of Community Health Centers serves as the collective voice for Pennsylvania's Federally Qualified Health Centers (FHQCs), FQHC Look-Alikes (FQHCLAs), and non-profit Rural Health Clinics (RHCs).  Our members are unique nonprofit primary care facilities that provide a wide array of services to Pennsylvanians in underserved areas of the state. There are nearly 300 sites in 52 counties serving over 800,000 Pennsylvanians.  At these primary care facilities, patients will receive high quality care at a price they can afford with sliding fees based on income and family size.

Focus: Diabetes

The Health Resources & Services Administration (HRSA) is aiming to replicate its successes with quality improvement and patient-centered medical home recognition to improve care, processes and outcomes by taking a similar approach to diabetes.  HRSA is urging health centers across the nation to increase focus and emphasis on prevention of diabetes and effective control and management of the illness when it occurs.  You will see a multipronged effort that includes new resources to support you in addressing this disease that is both costly in diminishing quality of life as well as costly to the healthcare system.  At least one out of seven people who walk through your health center door for care have diabetes - together, we can improve our effectiveness in preventing and managing this disease.
